CWC LIVEWELL & NHS PRIMARY CARE NEIGHBOURHOOD CARE CENTRES

In the last 8 years CWC (Independent Charity) has designed, built, and operated an integrated Well-Being, Illness Prevention, Social Prescribing Support Centre with a co-located NHS Primary Care Centre in Kew SW London delivering Wes Streeting’s vision for truly integrated, more effective and affordable Neighbourhood Care Centres. CWC was working on over 15 new Centres within commercial residential regeneration schemes in London when the current Government announced a root and branch reform of the NHS. The CWC LiveWell Project is now on “hold” pending the completion of the latest NHS reforms.

“I visited LiveWell Kew after a colleague said that she ‘had seen the future of health and social care integration and its name is ‘LiveWell Kew’”. My visit confirmed her impressions -- the co-location of primary care with services that can address the underlying causes of some physical and mental health issues is a model that holds great promise for creating a truly integrated care system.”
